Mason Risk Forms for 2024-2025:

You must complete all forms to participate in all EIP virtual and in-person events to remain in compliance with Mason’s policies for minors participating in EIP programming. 

Mason Risk Form

As you are completing the Mason Risk form packet, there are questions that that you must answer in the following way, please see below: 

Name of Camp Participant: Input the student name

Program/Camp Name: Early Identification Program

Location: George Mason University Campuses and online learning platforms used by EIP

Program Time(s): Time frame indicated by EIP

Camp: July 2024-June 2025

SOAR VA Enrollment Form

Enrollment is open to current 10th EIP students only. Reference email sent by EIP for details and the enrollment form to view required criteria for eligibility and pledge statements for SOAR VA participants.

 

FCPS Bus Permission Form

All FCPS EIP students must submit this form. All students were sent a paper letter with the Bus Permission Form via US Mail.  Student and Parent/Guardian must complete and return the form to their EIP Sponsor or School Counselor or fax it to 703-279-5211.
